Apply to Workability IV Program

To apply for the Workability IV Program, all students and alumni must complete and submit the following forms:

1. Workability Referral form
2. DOR Consent to Release and Obtain information (PDF)
3. DOR Employment Record (PDF)
4. DOR Health Questionnaire (PDF)

Completed forms should be brought to your first Department of Rehabilitation appointment.

If you are a current Fresno State student, you can meet with a DOR counselor at the Services for Students with Disabilities Office.  Please contact Troy Polnitz at 559.278.2811to schedule an appointment.

DOR Orientation

   Fresno -Tuesday 10:00 am 

   Visalia - Wednesday 10:00 am

   Merced - Wednesday 10:00 am

After your eligibility has been reviewed, you will be contacted to schedule a meeting with a DOR and WorkAbility Counselor.
